1. Eligibility for Admission

To be eligible for residency at Home for Elders, applicants must:

✔ Require assisted living, skilled nursing, or independent senior care.
✔ Be free from severe contagious diseases that may endanger other residents.
✔ Be medically assessed to determine care needs and compatibility with our facility.
✔ Provide all necessary medical and legal documentation for admission.


2. Admission Process

Our admission process ensures that each resident receives the care and support they need.

Step 1: Application Submission

✔ Complete and submit the admission application form.
✔ Provide medical records, emergency contacts, and personal information.

Step 2: Medical Evaluation & Assessment

✔ A full medical evaluation is required to assess the level of care needed.
✔ If necessary, we may schedule a meeting with the applicant’s doctor or family.

Step 3: Facility Tour & Agreement Review

3. Residency Guidelines & Expectations
